INITIAL NOTIFICATIONS

One of the most important parts of successful COBRA Administration is to ensure that each employee knows their rights.

A requirement of our plan is that each existing employee currently covered on your Benefits Plans gets an Initial Notification informing them that United Agencies is now administering your COBRA, and to contact us with any questions.

In order to do this we need some basic employee information to enter into our system, Name, SS#, Home Address, Date of Birth, etc.

Upon completion, please email this Spreadsheet to Jeff Gelineau for input into our system and generation of your Initial Notification letters.
